Job Summary
We are seeking a Home Care Case Manager As a Case Manager, you will play a crucial role in ensuring the highest level of care and service to our clients. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.  
 
Responsibilities 

  • Maintaining a fully staffed caseload at all times

  • Meeting all client needs and ensuring caregivers are appropriately matched and trained

  • Updating your supervisor on client birthdays within your caseload so a gift can be arranged

  • Ensuring client compliance with care plans, care assessments, and all necessary reports

  • Conducting 24-hour check-in calls following the start of service

  • Conducting a 10-day courtesy call with case managers after service initiation

  • Completing 30, 60, and 90-day reports for your clients

  • Performing bi-monthly home visits for quality assurance

  • Leading the initial intake process for new clients assigned to you

  • Ensuring care plans are updated annually for each client

  • Qualifications
  • High school diploma or GED
  • Previous experience as a Home Care Case Manager or in a similar position is preferred
  • Highly organized with attention to detail
  • Comfortable with Microsoft Office and other computer programs
  • Ability to multitask and prioritize projects
  • Excellent customer service and interpersonal skills
